Veterinary Anesthesia Consultation

Once you submit the form and make a payment, we will send you a request for more detailed information about your hospital and the patient, which will then be shared with a PEAK anesthesia consultant. Please fill out all requested information as completely as possible. This will allow us to make a detailed and thorough recommendation. If this is a STAT request and you do not have time to complete the secondary detailed set of information, the PEAK anesthesia representative will get the information from you during the consultation.

Our base consultation fee is $50.00 (USD).  Additional fees apply based on the requested response time and method.
